USJN Waiver & Concussion Policy

USJN BASKETBALL WAIVER

Athlete:
Any athlete who will be participating in a USJN tournament and/or a Blue Star basketball camp, skills academy, clinic, showcase, practice, or training must fill out and sign a Participant Waiver form. Any athlete under the age of 18 will need a parents/guardian signature in order for the waiver to be valid.

Team Events – Completed/signed waivers should be given to your team/club coach.
Individual Events – Please present your signed waiver upon registration for the event.

Coaches:
Coaches will need to provide a copy of each athlete’s signed Waiver at the Event registration. Once provided, we will input information into our database so the coach will not have to present at any other USJN or Blue Star event registration as long as the team roster does not change. If your team roster changes, the coach will need to provide us the signed waiver for the new player additions to the roster.

USJN BASKETBALL CONCUSSION POLICY

Any player suspected of having a concussion shall be immediately taken out of any event sponsored by USJN/Blue Star Basketball events (tournaments, camps, showcases, clinics, skills academy, training). A parent/guardian will be notified of the player’s condition. The player will not be allowed back for further involvement until evaluated and cleared in writing by medical personnel. The medical personnel must state in the letter that the individual has been cleared to play and allowed to participate in USJN/Blue Star Basketball events.

USJN/Blue Star Basketball has listed several Heads Up information sheets as a resource; however, USJN/Blue Star Basketball recommends individuals (athletes/parents/coaches) to review the Center of Disease Controls “Heads Up” resources on concussion awareness at:
